Do you Qualify?

The following questionnaire can be used to see if you (or the participant) meet the basic criteria required for participation in one of our CML clinical trials. Your answers to these questions will not be retained by this system. If answering these questions on behalf of someone other than yourself, you must be the legally authorized representative of the potential participant. If you (or the participant) do not meet the preliminary qualification, you may search for other clinical trials of interest or sign up to receive clinical trial updates.

1) Are you (or the patient) 18 years or older?

2) Have you (or the patient) been previously diagnosed with CML?
   Yes    No

3) Do you (or the patient) have CML in chronic, accelerated, or blast phase with a confirmed presence of the "Philadelphia" chromosome?
   Yes    No    Not Sure

4) Have you (or the patient) received therapy with imatinib (Gleevec^®), dasatinib (Sprycel^®) or nilotinib (Tasigna^®) and been told that you can no longer receive treatment with any one of these drugs or that these drugs may no longer be effective?

5) Do you (or the patient) have the T315I mutation associated with CML?

6) Do you (or the patient) have a history of either significant heart disease or HIV positive status?
